Four Easy Ways to Obtain a Birth or Death Record

There are four ways in which to obtain a birth or death record in Ohio. Choose the method that best suits your needs:

Local Vital Statistics Office - You can obtain birth and death records from the Zanesville-Muskingum County Health Department for events that occurred within Zanesville-Muskingum County, provided there is no court or legal action.

Please use the link to check the listing of local offices for the county of your birth or where death occurred. Locate the local health district office.

1.      Walk-in: Same day service is available to walk-in customers. This is the fastest way to obtain a birth or death certificate. When you arrive, you will complete an application and pay the $22.00 required fee. Customer Service Hours: Regular Office hours: Monday - Friday: 8:00 AM- 3:50 PM (Note: We are closed all Federal Holidays)

We accept cash, check, or money orders (in U.S. funds)

We do not accept credit/debit cards in office.

Approximately wait time is 10-15 minutes.

No request will be filled if the application is not complete. Use the VS application tab on the menu to download an application.

2.      Mail: - Turn around: SAME DAY. The day we receive the request (Use the VS application tab on the menu to download an application.) the document is processed and sent out in that evening’s mail. Please enclose a self-addressed stamped envelope for us to return your request to you. You are urged to allow sufficient time for delivery for all birth/death records. Mail money order of $22.00 for each certificate. We do not accept credit/debit cards by mail.

3.      At the State Department of Vital Statistics
