Take The Community Health Survey!

The Southeastern Ohio Health Improvement Collaborative (Genesis HealthCare System, Morgan County Health Department, Noble County Health Department, Perry County Health Department, and Zanesville-Muskingum County Health Department) is conducting a Community Health Needs Assessment (CHNA) to identify the needs of the community. We are asking community members (those who live and/or work in these counties) to complete this short, 20-minute survey. We need 400 responses by October 31st. This information will form the next Community Health Improvement Plan that community partners will be working on to improve our community.
